PROOF OF UPDATED COVID VACCINES IS REQUIRED
FOR ENTRY TO IN-PERSON WORSHIP.

 

Please upload a copy of your vaccine card below to be kept on file or be prepared to show it in person upon arrival.

Masks are not required but are strongly recommended.

If you are not fully up to date on vaccines  OR if you have tested positive within the past 10 days, we welcome you to join us on Zoom.
